My First Visit

What will happen at My First Visit?

We will review your medical and dental health history and perform a thorough periodontal examination to access bone loss, loose teeth, bite problems, oral cancer, TMJ (jaw joint) problems, missing teeth, bleeding gums, and other indicators of periodontal disease. We will also perform a smile analysis and evaluate ways to use dental implants to replace any missing teeth.

Please allow approximately 90 to 120 minutes for your initial visit.

What is Needed for My First Visit?

Please assist us at the time of your initial visit to the office by providing the following information:

  1. Your referral form and x-rays from your referring dentist.
  2. A list of medications you are currently taking.
  3. A list of medical doctors your currently see with their phone numbers.
  4. Fully Completed New Patient and Health History Forms prior to your appointment.
    (These can be filled out online, downloaded and filled out, E-mailed (email), mailed, or faxed.) Click here to Register as a New Patient.
  5. If you have dental insurance, please bring any insurance forms or insurance cards with you to the appointment. (We must verify your insurance to help you obtain your benefits).

Will my initial exam hurt?

We will be as gentle as possible. The periodontal exam can be completed with little or no discomfort. If your gums are exceptionally sensitive, you can use Magic Mouth Rinse (a numbing medication)(link) to make the exam more comfortable. For the extremely anxious patient, we can also use nitrous oxide (laughing gas) to keep you relaxed and comfortable.

Do I Need X-Rays?

We will need current periodontal x-rays (link) in order to see disease not otherwise visible. We also need x-rays for dental implant procedures. If your referring dentist has taken x-rays, you may request that they be forwarded to us. If you like, your x-rays, etc can be E-mailed to our office at info@PerioMem.com. We use digital x-rays to keep your radiation exposure to a minimum.
